The relationship between clinical presentation and the nature of care in adults with intellectual disability and epilepsy – national comparative cohort study

Background
A quarter of People with Intellectual Disabilities (PwID) have epilepsy compared with 1% of the general population. Epilepsy in PwID is a bellwether for premature mortality, multimorbidity and polypharmacy. This group depends on their care provider to give relevant information for management, especially epilepsy. There is no research on care status relationship and clinical characteristics of PwID and epilepsy.
AimExplore and compare the clinical characteristics of PwID with epilepsy across different care settings.
MethodA retrospective multicentre cohort study across England and Wales collected information on seizure characteristics, intellectual disability severity, neurodevelopmental/biological/psychiatric comorbidities, medication including psychotropics/anti-seizure medication, and care status. Clinical characteristics were compared across different care settings, and those aged over and younger than 40 years.
ResultsOf 618 adult PwID across six centres (male:female = 61%:39%), 338 (55%) received professional care whereas 258 (42%) lived with family. Significant differences between the care groups existed in intellectual disability severity (P = 0.01), autism presence (P < 0.001), challenging behaviour (P < 0.001) and comorbid physical conditions (P = 0.008). The two groups did not vary in intellectual disability severity/genetic conditions/seizure type and frequency/psychiatric disorders. The professional care cohort experienced increased polypharmacy (P < 0.001) and antipsychotic/psychotropic use (P < 0.001/P = 0.008).
The over-40s cohort had lower autism spectrum disorder (ASD) and attention-deficit hyperactivity disorder (ADHD) comorbidity (P < 0.001/P = 0.007), increased psychiatric comorbidity and challenging behaviour (P < 0.05), physical multimorbidity (P < 0.001), polypharmacy (P < 0.001) and antipsychotic use (P < 0.001) but reduced numbers of seizures (P = 0.007).
ConclusionPwID and epilepsy over 40 years in professional care have more complex clinical characteristics, increased polypharmacy and antipsychotic prescribing but fewer seizures.

Background
Brain in Hand (BIH) is a UK-based digital self-support system for managing anxiety and social functioning.
AimsTo identify the impact of BIH on the psychological and social functioning of adults with autism.
MethodAdults with diagnosed or suspected DSM-5 (level 1) autism, identified by seven NHS autism services in England and Wales, were recruited for a 12-week prospective mixed-methods cohort study. The primary quantitative outcome measures were the Health of the Nation Outcome Scales for People with Learning Disabilities (HONOS-LD) and the Hospital Anxiety and Depression Scale (HADS). Fisher's exact test explored sociodemographic associations. Paired t-test was utilised for pre–post analysis of overall effectiveness of BIH. Multivariable linear regression models, univariable pre–post analysis, Wilcoxon signed-rank test, logistic regression analysis, Bonferroni correction and normative analysis were used to give confidence in changes identified. A thematic analysis of semi-structured exist interviews following Braun and Clarke's six-step process of 10% of participants who completed the study was undertaken.
ResultsSixty-six of 99 participants completed the study. There was significant reduction in mean HONOS-LD scores, with 0.65 s.d. decrease in those who used BIH for 12 weeks. Significant positive changes were identified in HONOS-LD subdomains of ‘self-injurious behaviours’, ‘memory and orientation’, ‘communication problems in understanding’, ‘occupation and activities’ and ‘problems with relationship’. A significant reduction in the anxiety, but not depression, component of the HADS scores was identified. Thematic analysis showed high confidence in BIH.
ConclusionsBIH improved anxiety and other clinical, social and functioning outcomes of adults with autism.

The draft Mental Health Bill, which amends the Mental Health Act 1983 for England and Wales, proposes protections for people with intellectual disability and/or autism (ID/A) to prevent detention in hospital in the absence of mental illness. This editorial critically appraises the positive impact and unintended consequences of the proposed reforms for people with ID/A.

Background
An increasing number of children, adolescents and adults with intellectual disabilities and/or autism are being admitted to general psychiatric wards and cared for by general psychiatrists.
AimsThe aim of this systematic review was to consider the likely effectiveness of in-patient treatment for this population, and compare and contrast differing models of in-patient care.
MethodA systematic search was completed to identify papers where authors had reported data about the effectiveness of in-patient admissions with reference to one of three domains: treatment effect (e.g. length of stay, clinical outcome, readmission), patient safety (e.g. restrictive practices) and patient experience (e.g. patient or family satisfaction). Where possible, outcomes associated with admission were considered further within the context of differing models of in-patient care (e.g. specialist in-patient services versus general mental health in-patient services).
ResultsA total of 106 studies were included and there was evidence that improvements in mental health, social functioning, behaviour and forensic risk were associated with in-patient admission. There were two main models of in-patient psychiatric care described within the literature: admission to a specialist intellectual disability or general mental health in-patient service. Patients admitted to specialist intellectual disability in-patient services had greater complexity, but there were additional benefits, including fewer out-of-area discharges and lower seclusion rates.
ConclusionsThere was evidence that admission to in-patient services was associated with improvements in mental health for this population. There was some evidence indicating better outcomes for those admitted to specialist services.

Background
Transcranial direct current stimulation (tDCS) could be a side-effect-free alternative to psychostimulants in attention-deficit/hyperactivity disorder (ADHD). Although there is limited evidence for clinical and cognitive effects, most studies were small, single-session and stimulated left dorsolateral prefrontal cortex (dlPFC). No sham-controlled study has stimulated the right inferior frontal cortex (rIFC), which is the most consistently under-functioning region in ADHD, with multiple anodal-tDCS sessions combined with cognitive training (CT) to enhance effects. Thus, we investigated the clinical and cognitive effects of multi-session anodal-tDCS over rIFC combined with CT in double-blind, randomised, sham-controlled trial (RCT, ISRCTN48265228).
MethodsFifty boys with ADHD (10–18 years) received 15 weekday sessions of anodal- or sham-tDCS over rIFC combined with CT (20 min, 1 mA). ANCOVA, adjusting for baseline measures, age and medication status, tested group differences in clinical and ADHD-relevant executive functions at posttreatment and after 6 months.
ResultsADHD-Rating Scale, Conners ADHD Index and adverse effects were significantly lower at post-treatment after sham relative to anodal tDCS. No other effects were significant.
ConclusionsThis rigorous and largest RCT of tDCS in adolescent boys with ADHD found no evidence of improved ADHD symptoms or cognitive performance following multi-session anodal tDCS over rIFC combined with CT. These findings extend limited meta-analytic evidence of cognitive and clinical effects in ADHD after 1–5 tDCS sessions over mainly left dlPFC. Given that tDCS is commercially and clinically available, the findings are important as they suggest that rIFC stimulation may not be indicated as a neurotherapy for cognitive or clinical remediation for ADHD.

Construction is by far the largest material-consuming activity in the UK. In 2016, the territory produced more than 800 kg of construction and demolition (C&D) waste per person.1 Most of these building materials are energy intensive to produce and environmentally costly to transport. Their premature replacement therefore contributes significantly to the increasing levels of greenhouse gases in the atmosphere.

A metric was developed to identify hospital proportion of carbapenem consumption (PoCC) among antipseudomonal antibiotics. The PoCC varied significantly among academic medical centers by Census Bureau geographic division after adjusting for patient mix. This metric may be useful in identifying disproportionate carbapenem use and potential carbapenem overuse.

Crevasse initiation is linked to strain rates that range over three orders of magnitude (0.001 and 0.163 a-1) as a result of the temperature-dependent nonlinear rheological properties of ice and from water and debris inclusions. Here we discuss a small cold glacier that contains buried crevasses at and near an ice divide. Surface-conformable stratigraphy, the glacier’s small size, and cold temperatures argue for limited rheological variability at this site. Surface ice-flow velocities of (1.2-15.5) ± 0.472 m a- 1 imply classic saddle flow surrounding the ice divide. Numerical models that incorporate field-observed boundary conditions suggest extensional strain rates of 0.003-0.015 a- 1 , which fall within the published estimates required for crevasse initiation. The occurrence of one crevasse beginning at 50 m depth that appears to penetrate close to the bed suggests that it formed at depth. Field data and numerical models indicate that a higher interior stress at this crevasse location may be associated with steep convex bed topography; however, the dynamics that caused its formation are not entirely clear.

The science of extra-solar planets is one of the most rapidly changing areas of astrophysics and since 1995 the number of planets known has increased by almost two orders of magnitude. A combination of ground-based surveys and dedicated space missions has resulted in 560-plus planets being detected, and over 1200 that await confirmation. NASA's Kepler mission has opened up the possibility of discovering Earth-like planets in the habitable zone around some of the 100,000 stars it is surveying during its 3 to 4-year lifetime. The new ESA's Gaia mission is expected to discover thousands of new planets around stars within 200 parsecs of the Sun. The key challenge now is moving on from discovery, important though that remains, to characterisation: what are these planets actually like, and why are they as they are?
In the past ten years, we have learned how to obtain the first spectra of exoplanets using transit transmission and emission spectroscopy. With the high stability of Spitzer, Hubble, and large ground-based telescopes the spectra of bright close-in massive planets can be obtained and species like water vapour, methane, carbon monoxide and dioxide have been detected. With transit science came the first tangible remote sensing of these planetary bodies and so one can start to extrapolate from what has been learnt from Solar System probes to what one might plan to learn about their faraway siblings. As we learn more about the atmospheres, surfaces and near-surfaces of these remote bodies, we will begin to build up a clearer picture of their construction, history and suitability for life.
The Exoplanet Characterisation Observatory, EChO, will be the first dedicated mission to investigate the physics and chemistry of Exoplanetary Atmospheres. By characterising spectroscopically more bodies in different environments we will take detailed planetology out of the Solar System and into the Galaxy as a whole.
EChO has now been selected by the European Space Agency to be assessed as one of four M3 mission candidates.

Summary
The Small Pelagic Fish and Climate Change (SPACC) program was created to facilitate research on the dynamics of populations of small pelagic fish, including anchovy and sardine. These populations exhibit large variations in size, extent, and production on the scale of decades. At times, anchovy and sardine alternate in abundance. Collectively, small pelagic fish often occupy a central role in the food web they occur in, often described as a wasp-waist ecosystem. Humans are an integral part of those ecosystems. Variability of populations of small pelagic fish is believed to be due primarily to variations in climate and fishing, but the mechanisms of these relations remain unknown in most cases. It is also uncertain whether these ecosystems alternate between states, e.g. regimes, and whether inherent variability may limit our ability to predict their future states. The fisheries for populations of small pelagic fish are increasingly global in nature. While the global catch of small pelagic fish constitutes approximately one quarter of the world fish catch and has been relatively constant during the past several decades, the catch of individual taxa and stocks varies much more. The management of these fisheries will be challenged by increasing demand for human consumption and mariculture in light of their finite and variable production, importance within the ecosystem, and unprecedented climate change, and will depend on both science and governance. We recommend continued, global research on climate change effects on small pelagic fish, and its periodic assessment for use by decision makers.
AMPA-preferring receptors mediate excitatory non-NMDA responses of primate retinal ganglion cells

Glutamate and kainate-induced currents of primate ganglion cells were studied using the whole-cell patch-clamp technique in a retinal slice preparation. Antagonists and allosteric modulators of desensitization selective for either α-amino-3-hydroxy-5-methyl-4-isoazoleprionic acid (AMPA)- or kainate-preferring receptors were used to determine the contributions of each type of receptor to excitatory responses. With synaptic transmission and NMDA receptors blocked, the AMPA-preferring receptor antagonist GYKI 52466 (30 μM–100 μM) reversibly blocked most of the glutamate-induced current in ganglion cells. GYKI 52466 also blocked the response in ganglion cells to focally applied kainate, suggesting that the current response to kainate arises from activation of AMPA-preferring receptors, and not kainate-preferring receptors. Both cyclothiazide (10 μM–100 μM) and the novel drug 4-[2-(phenylsulfonylamino)ethylthio]-2,6-difluoro-phenoxyacetamide (PEPA, 20 μM–100 μM), which selectively enhance responses mediated by AMPA-preferring receptors, enhanced glutamate-induced responses of ganglion cells. Since these drugs preferentially inhibit desensitization of the flip and flop splice variants, respectively, of AMPA-preferring receptors, it is likely that both splice variants are present on these ganglion cells. Concanavalin A, which selectively suppresses the desensitization of kainate-preferring receptors, had no effect on the glutamate-induced responses of ganglion cells. We conclude that the non-NMDA component of the excitatory, glutamatergic input to primate ganglion cells is mediated largely by AMPA-preferring receptors, with little, if any, kainate-preferring receptor-mediated response.

Shells of agglutinated foraminiferan protists are composed of mineral grains bound by secreted adhesives. As such, they are useful models for examining the evolution of “primitive” exoskeletons. Previous studies revealed the ultrastructure of shells in the giant Antarctic foraminiferan Astrammina rara and demonstrated that shucked specimens would reconstruct shells using glass beads. Here we further investigate shell architecture in this model species.
For micromechanical testing, an intact A. rara shell was placed between a fixed plate and a facing plate in series with a calibrated load cell. Displacement was effected by a high-precision drive, and 2-3 loading cycles were used to determine shell material properties. to assay tensile properties of the adhesive matrix, a network of pseudopodia and extracellular matrix fibers (i.e., the shell adhesive component) was obtained by incubating shucked cell bodies on 200-mesh gold grids. Pseudopodia were subsequently removed by detergent washes. Fibers in the resultant isolated matrix were severed with a Nd: YAG laser using an inverted DIC light microscope equipped with a 60× objective lens. Preliminary loading experiments using glass needles showed that Sepharose 2B beads were suitable strain gauges to assess compression within reconstructed shells (Fig. 1). in this assay, shucked cell bodies were incubated with a mixture of glass and Sepharose beads, and the reconstructed shells were examined by SEM.
Repeated loading and unloading demonstrated the elastic behavior of intact shells (Fig. 2). Adhesive matrix fibers snapped towards their attachment sites within 2 sec after cutting with a laser (Fig. 3), demonstrating that they are deployed under tension. SEM images of shells reconstructed with Sepharose show compressed particle profiles (Fig. 4).
Strengthening of Aluminum by Oxygen Implantation: Experimental Results and Mechanical Modeling

The microstructuTe and mechanical properties of high purity aluminum implanted with 20 at.% oxygen to a depth of roughly 500 nm and subjected to various thermal histories have been examined. Transmission electron microscopy and Rutherford-backscattering spectrometry were used to characterize the depth and nature of the implanted zone. As implanted, the material appears to contain a homogeneous distribution of disordered precipitates with sizes of 1.5-3.5 nm. Annealing at 450 or 550ΰC for 1 hour, induces ordering of the precipitates but only causes slight coarsening. Ultra-low load indentation hardness testing was used to probe the mechanical response of the surface-modified material. The data from the hardness tests were interpreted through the use of a finite-element model; the results indicate the flow stresses of an implanted and annealed layer are as high as 1600 MPa. The as-implanted material is much harder, approaching 3300 MPa. The degree of strengthening expected for the as-implanted and post-annealed material on the basis of the observed microstructure was estimated using several micromechanical models, and the results conform to the findings from indentation testing.

Nel 1967 e nel 1968 sono state svolte, in Francia ed in Italia, delle indagini post-elettorali, a livello nazionale, su campioni di opinione pubblica, nell−ambito del medesimo progetto per uno studio comparato fra nazioni sul tema del governo rappresentativo. In questa sede, riferiamo su alcune conclusioni tratte da una serie piú ristretta di dati relativi al rapporto, riscontrabile nei due paesi, fra l−opinione espressa dal pubblico su problemi scelti e le sue preferenze politiche.
